City of Palatka issued the following announcement.
If you are a healthcare worker with direct patient contact or are age 65 and over and are interested in getting a Moderna COVID-19 vaccine through the Putnam County Health Department, please fill out the form on the Putnam County Health Department's website link below. The Moderna COVID-19 vaccine is administered as a 2-dose series, 1 month apart. If you have already completed this form, there is NO NEED to re-submit or call the hotline. If you have an difficulties completing this form please call the Putnam County COVID Hotline at (386) 329-1904 Option #3
